In support of our new special Pikeville Medical Center and AVA Center license plate, for those interested in showing your PMC support, PMC will cover the initial purchase price of $44.00 for the first 500 vehicles of employees, board members, staff, and AVA Center parents who sign up by submitting this form to have this license plate placed on their vehicle. PMC is only covering the first initial annual fee for this plate for those who sign up. Future annual fees and expenses will be covered by the vehicle owner. If you are interested in having one or more of these awesome license plates, to show your PMC pride and support for our amazing AVA Center and our Learners, please provide your contact information below so that we may add you to our list of those who wish to take advantage of this offer. Also note, that $10 for each plate annually will go to the AVA Center in support for this amazing service.

Once you fill out the form below, we will follow up giving you instructions on how to proceed with picking up your plate from  your county clerk’s office. This will be in the county clerk where you vehicle is currently registered. When picking up your new plate(s) you will need to make sure that you take a copy of your insurance card and your old license plate.

For those in Pike and Floyd Counties, your name will be placed on a list and the plate will be provided free of charge. If your vehicle is registered in a different county, you will need to first pay for the new special plate. Submit a copy of the receipt to the following email address, cindy.thieme@pikevillehospital.org, and PMC will gladly reimburse you.

Again, as a special offer to the first 500 plates sold, PMC has agreed to cover the initial $44.00 annual cost of the basic license plate. If you elect to customize your new PMC special plate, this additional customization cost will be the owners responsibility along with any other additional costs or expenses including but not limited to the titling, registering, or insuring your car.

PMC is excited to extend this limited special offer for the first 500 plates.  Please fill out the attached form.  With the interest, the 500 initial plate offer will not last long.  If however, after the initial 500 plates are sold, anyone including the general public, can still request these special plate, but will be required to cover the $44.00 fee.  We are looking forward to seeing the pride of PMC and support for those on the spectrum displayed throughout our region in the coming weeks and years.
